Population Health 2.0

Registration is closed for this event
Are you "SILOed"? Population health like you've never seen it before, is the focus of a new comprehensive, landmark book, “Population Health,” written by local and national authors.

Population Health 2.0
For the 3rd time, the Health Enterprises Network addresses this "paradigm-shifting" and "career-changing" issue.

There is great focus on population health to shape the mandated improvements in providing greater care organization across all care settings and delivery methods, while navigating new reimbursement policies. The keys to success lie in innovation, collaborative approaches, ACO strategies, patient centered medical homes and the integration of technology and data.

Population health like you've never seen it before, is the focus of a new comprehensive, landmark book, “Population Health,” penned by at total of 28 local and national authors.

Topic presentations will include advancing personal health data, promoting wellness through systems and policy changes, and improving population through health collaborative networks, with a special emphasis on aging and the continuum of care. A formal presentation of an autographed book will be made to Mayor Greg Fischer, Louisville Metro Government, by Craig H. Blakely PhD, MPH, professor and dean, School of Public Health & Information Sciences, UofL.
